GENERAL TERMS AND CONDITIONS APPLICABLE TO PERSONS JOINING THIS PRACTICE AS PATIENTS

PRIVACY INFORMATION
The personal and health information that is provided by you and recorded in your Electronic Health Record (Cliniko) is collected to provide you with treatment. Your electronic medical record will be stored by the practice for as long as this practice is operating after which you will be contacted for instructions regarding your medical records. Your information is also stored in our mailing service and WhatsApp messages. All the information provided by you is treated as strictly confidential by Dr Novikova and the practice staff. When you submit your invoice to medical aid for reimbursement, it will contain your diagnosis (ICD-10 code) which may be disclosed to the main member of the medical (on your plan). During the procedures, photos and videos may be taken to be included in your health record. Your personal information may be shared with other health professionals participating in your care with your consent. 
 
PRACTICE COMMUNICATION 
Your information, test results, appointment confirmation will be communicated with you via emails and WhatsApp messages. Your test results will be sent to you via e-mail and you are expected to acknowledge them or contact us if you did not receive your test results. 
We do not provide email consultation. You are welcome to book video consultations. Online bookings via the website are available. 
You may receive monthly newsletter/updates from the practice.
Repeat prescriptions, medical reports, past results and referrals are charged separately. 
 
TESTS & FEES INVOLVED
Fees for any laboratory tests (including Pap smear, HPV test, STDs tests, mammograms, any blood tests) are not included in Dr Novikova’s fees. Dr Novikova’s practice works with SaPath, Pathcare, Lancet and your lab account will be sent to your medical aid or you directly. 
 
HOSPITAL PROCEDURES
If hospitalisation is required, it remains your responsibility to ensure that the planned treatment is covered by your medical aid and that the necessary finances are arranged to cover non-insured costs. The medical aids do not cover aesthetic surgery. Medical aid may sometimes cover part of your treatment and you are responsible to cover the gap. The practice will provide you with all the necessary documentation to facilitate the authorisation process. This practice does NOT deal with medical aids on your behalf. 
 
SETTLING OF ACCOUNTS
Our fees are based on an independent survey of practice costs within the industry and may be viewed on our website. Office consultations and procedures are due and payable in full on the day of service. Surgery fees a week before the surgery date. Current payment options are credit/debit cards, cash and EFT.
DR NOVIKOVA’s PRACTICE IS NOT CONTACTED TO ANY MEDICAL AID. Patients are responsible for their bills payment at the time of service and the practice will provide the patient with a receipt to claim from medical aid. Dr Novikova’s practice has no responsibility in terms of the amount refunded by medical aid to the patient. 
 
EMERGENCY MEDICAL CARE
Dr Novikova only provides emergency gynaecological care to the patients in her practice and not new patients. You should contact the nearest hospital for assistance.
